The term "connected health" reflects the range of opportunities for technology-enabled care programs and the potential for new strategies in health care delivery.
Partners HealthCare Connected Health, develops innovative and effective solutions for delivering quality patient care outside of the traditional medical setting. The Center engages in pioneering research in a wide range of connected health-related areas and works to advance the field through its convening and publishing activities.
Our Connected Health programs use a combination of remote-monitoring, wireless, and online communications and intelligence to improve patient adherence, engagement and clinical outcomes. Participants in our programs are patients and providers at Partners-affiliated practices and hospitals throughout New England.
In addition the Connected Health Symposium brings together more than 1,000 health care professionals each year to Boston to discuss and debate the means of moving health care out of the clinic setting and into the day-to-day lives of those who need care.
We are engaging patients, providers and the connected health community to deliver quality care outside of traditional medical settings. Telehealth, mobile health (mHealth), remote care and disease management initiatives reflect the opportunities for technology-enabled care programs.
