Partners HealthCare is aiming to train all primary care physicians in the system by 2020 to help patients make end-of-life care decisions.

The Palliative Care+ project is a collaboration between North Shore Physicians Group and Care Dimensions, a palliative and hospice care nonprofit. The Partners Medicare Accountable Care Organization program enables primary care providers to talk to patients who have serious illnesses about their wishes regarding life-sustaining treatment, health care proxies, and pain management prior to emergencies.

In addition, nurses monitor the patients’ daily vitals to avoid hospitalizations and emergency department visits, which is expected to save $7.2 million during the project.
