The BWH Department of Physician Services and the MGH Registrars
Office are a resource for administrative matters relating to your
hospital appointment. These offices can also provide information and
assistance in the areas listed below:

NOTE: It is strongly recommended that
physicians keep their own files containing copies of all paperwork
related to credentialing, hospital appointment, licensure, contracts,
etc., as photocopies of the following documents may be requested by
the MGH Registrar, the BWH Physician Services Office and other departments
and/or hospitals, as needed (e.g., for rotations to outside institutions):
- Massachusetts medical license application
- certificate of limited registration, or wallet card license (if
full license)
- any other state medical licenses, and applications for same
- malpractice proof of insurance facesheet with coverage dates
and limits
- medical school diploma (make copies before it
is framed)
- federal DEA certificate and the DEA application
- Massachusetts Controlled Substances Certificate (MCSC), and the
MCSC application, until the certificate is received
- ECFMG certificate, if applicable
- original scores from USMLE Steps 1, 2 and 3
- certificates of board certification, when obtained
Please note: Administrative support for ECFMG and
limited licensing issues for a trainee is provided at MGH by the Registrar/Credentialing
Unit and at BWH by the Department of Physician Services. The MGH Registrars
office is located in the Resident/Physician House on the MGH main
campus. Donna Downs and Dale Parechanian are the primary contacts
for licensing they can be reached at (617) 726-2119. Physician
Services is located at 850 Boylston Street. Kate Annis is the primary
contact and can be reached at (617) 732-9437. In addition, the Partners
International Office, specifically Clare Ayer at (617) 724-3969, remains
an excellent resource for questions regarding H1-B, Os, J-1
Research and F-1 visas.
The MGH Registrar/Credentialing also provides
the following services:
- Professional Staff with Statements certifying title and dates of
all appointments held at MGH. Professional staff members may also
request letters of certification to be sent to third parties.
- Residents and Clinical Fellows eligible for loan deferments may
send or drop off deferment forms to the Registrar/Credentialing Unit
for institutional authorization. Deferment forms must be obtained
from lending institutions. Eligibility for deferment is determined
by the lender. Deferment forms should be completed by the borrower
and clearly indicate the appropriate mailing address. Once the institution
authorization is complete, deferment forms will be mailed directly
to the appropriate agency and a copy will be mailed to the borrower.
Please note: copies of deferments are NOT kept on file in the office.
- Notary Services are available to the MGH/MGPO professional Staff,
at no charge, during office hours, 8:30 am
5 pm (Monday Friday). Please bring a picture ID and original
documents for notarization. Please call ahead to make sure notary
