Q: Why should these documents be kept in the binder?

Keep this updated information on file to document the competency of all lab facilities being used, and to support the reliability of test results.

Q: What should be done if these laboratory documents are filed in a different location?

Write a signed and dated note-to-file indicating the location of these documents.

Q: We use a research lab for our specimens. What should we have on file?

Research laboratories typically do not have lab certifications (e.g. CLIA) and may not have “normal” lab values. In the case of research laboratories, ensure that the lab director's CV and research lab reference values are on file.
