The three D’s of the Smart Form are:
Smart Form is designed for use by clinical providers. It is envisioned to be integrated in various workflows and be used before, during, after and in between patient visits. While primarily targeted as a documentation tool, Smart Form will additionally support a mode of interaction focused mainly on data review and interacting with decision support components. There is only one Smart Form. Smart Form content for a condition or a combination of conditions will be developed independently by clinical experts, and presented to the user in a unified fashion, on one screen, using consistent user interface elements to the extent possible. Each set of condition-specific content is referred to as a Smart Form template. A template may contain information on documentation, data review and decision support pertinent to the condition. If patient meets the qualifying conditions, information from multiple condition-specific templates is intelligently merged into one Smart Form for presentation. |
