Today, Partners Continuing Care and its affiliated provider network includes:

  • A Home Care organization that provides a full array of Certified, Specialty, Life-Line and Private Duty home care services to over 27,000 patients and their families in over 140 towns throughout eastern Massachusetts;
  • 4 Specialty Rehabilitation and Long Term Acute Care Hospitals with 656 beds that serve over 8,000 inpatients per year;
  • 21 Outpatient Centers that provide over 240,000 outpatient visits per year;
  • 5 Rehabilitation Units with 201 additional short-term inpatient beds that provide services through management contracts;
  • 4 Short-term Skilled Nursing Facilities / Units with 277 beds that provide non-hospital level transitional care, as well as 60 beds dedicated to residential care;
  • 1 Retirement Community with Senior Housing and an Adult Assisted Living Facility on campus.
