Waltham, MA – Partners HealthCare at Home (PHH) announced the appointment of Rod Carnifax as its new president who will serve as its senior advisor on strategy, planning, and the integration of opportunities that will grow PHH’s business and reputation throughout Massachusetts. In this role, he will work closely with clinical and administrative leaders throughout Partners and the region to develop new models of home healthcare that will grow in importance in the coming years

Carnifax, PT, MBA, came to Partners from Chicago where he served as vice president of home health services and palliative care at Presence Health, Illinois’ largest Catholic health system formed by the merger of Provena Health and Resurrection Health Care. Throughout his time in Chicago, he participated in the development and implementation of strategies to prepare Presence Health for the era of value based health care and population health management. He collaborated with hospital executives and system leaders to integrate acute and post-acute care protocols and also led multidisciplinary care initiatives to enhance transitions of care, increase patient satisfaction, and improve outcomes.
