Connected Cardiac Care
Heart failure is a chronic and difficult disease to manage. In fact it is the number one reason for hospital admission among people over 65 years of age. The Partners HealthCare Center for Connected Health is developing new ways to help patients manage heart disease by integrating technology into patient care and supporting self-monitoring.
The Connected Cardiac Care program uses technology to help patients and their physicians manage their heart failure from their own home. By changing the way patients manage their heart failure, Partners is helping patients live more healthy lives and avoid hospitalization.
Connected Cardiac Care allows patients to monitor their physiological health on a daily basis and provides a link to their health care team from their home. The Partners Connected Cardiac Care team brings easy-to-use equipment to a patient’s home, installs it, and shows the patient how to use it. There is no cost to enroll or for use of the equipment.
The goals of this four-month program are to help patients better understand this disease and be able to make changes to their lifestyle to prevent hospitalizations.
