The health care world is changing. Clinicians and hospitals are being asked to do everything from demonstrating meaningful use of electronic health records to accepting financial risk for patient services to managing entire patient populations as the government and marketplace focus on the affordability of care and quality improvement.
Through its strategic initiatives, Partners is redesigning care delivery to improve the health of our patients while reducing the cost of care. Achieving these goals also requires a level of coordination and standardization across Partners based on a fully-integrated electronic health record system.
In July 2012, Partners HealthCare announced it would partner with Epic, the industry-leading provider of health information technology to develop and implement an integrated, electronic health information system at all institutions across the Partners network by 2017. This initiative, Partners eCare, is the largest program of its kind in the history of Partners HealthCare.
Supporting the Partners Mission
Partners eCare is supporting Partners’ innovation and leadership in redesigning patient care models, advancing population health management, improving patient affordability, enhancing the patient experience, and strengthening community-based care. Partners eCare will help Partners fulfill its pledge to deliver the highest quality care to patients that is safe, effective, accessible, and affordable.
Building this new, unifying system is enabling Partners to be a more agile, integrated, and efficient health care system, ready to meet the challenges of state and federal health care reform for years to come. It is an investment that supports our publicly-stated goal of tackling health care’s toughest challenges, and it further realizes our founding hospitals’ original commitment to add value to health care by working together as a system.
At Its Core, a New Information Infrastructure
The Partners eCare guiding principle is “one patient, one record, one team, one Partners statement” enabling a seamless flow of clinical and administrative information about a patient anywhere within the Partners system. The goal is to improve coordination, enhance patient care quality and safety, and support our nation’s urgent healthcare quality and cost imperatives.
As Partners eCare has rolled out across our system, it is supported by an information technology (IT) infrastructure that:
- connects the continuum of care from primary, specialty, and acute hospital care to rehabilitation and home care;
- engages patients in their own care;
- improves access to the knowledge and information that advances our research and teaching missions; and
- supports Partners’ ongoing clinical, financial and regulatory analysis and reporting.
Involving All Staff, Reaching Every Patient
The clinical component of Partners eCare will help patient care teams to better manage high-risk patients, reduce readmissions, and better coordinate the full continuum of care across our sites and services. The administrative component of Partners eCare will help improve the efficiency of patient registration, scheduling, and billing. Together, they will serve as the foundation for innovative new care models that Partners HealthCare is pioneering today.
