The Center for Connected Health, a division of Partners HealthCare, develops innovative and effective solutions for delivering quality patient care outside of the traditional medical setting, including to patients’ homes. The term “connected health” refers to a model of healthcare delivery that uses technology to provide healthcare remotely.
