Connecting Patients with Primary Care

Partners is working to ensure that patients get the right care at the right place. Since October 2005, hospital staff have worked with patients who come through our emergency and other hospital departments to connect them to community-based primary care providers who ensure they receive ongoing comprehensive care.

To date, more than 8,900 patients have been connected to primary care through the work of staff at Brigham and Women’s Hospital, Massachusetts General Hospital, and North Shore Medical Center.

“When I can place a patient in a primary care setting when they need it, at the time they need it, and in a language that they can understand — that’s my best day.”

- Josefina Roques, a primary care access coordinator at Brigham and Women’s Hospital (BWH).

Being able to assist patients from every ethnicity, socioeconomic status and cultural background, to find a primary care physician they are comfortable with, makes doing this work a pleasure more than a task.”

- Bernice McField-Avila, primary care access coordinator at Mass General Hospital (MGH)
