As a key part of Partners integrated health care delivery system,
Partners Continuing Care is dedicated to providing a comprehensive
array of rehabilitation, long term acute care, skilled nursing and
home health care services to patients and their families.

Today, Partners
Continuing Care and its affiliated provider network includes:
- A home care and hospice organization that provides a full array of home health, hospice, private care, and Lifeline that enhance quality of life. Serving Eastern Massachusetts;
- 4 Specialty Rehabilitation and Long Term Acute Care Hospitals
with 656 beds that serve over 8,000 inpatients per year;
- 21 Outpatient Centers that provide over 240,000 outpatient visits
per year;
- 5 Rehabilitation Units with 201 additional short-term inpatient
beds that provide services through management contracts;
- 4 Short-term Skilled Nursing Facilities / Units with 277 beds
that provide non-hospital level transitional care, as well as 60
beds dedicated to residential care;
- 1 Retirement Community with Senior Housing and an Adult Assisted
Living Facility on campus.
